At Kiwi Vet Behaviour we are committed to providing the best possible outcomes! Our training and recommendations will always prioritise the safety and wellbeing of your pet and household. Please read the following carefully. If you have any questions please contact the team at Kiwi Vet Behaviour to discuss before completing and agreeing to our terms & conditions.*

I have provided accurate information to the best of my knowledge and have not purposely withheld any information regarding my pet's health or behaviour. I agree to update the Kiwi Vet Behaviour team with any changes in my pet's health or behaviour that may have an impact on the safety of my own pet and/or other animals and people.*

I understand that much of my pet's learning and progress will occur in between and outside of formal training sessions and for this reason the Kiwi Vet Behaviour team cannot guarantee any training and behavioural outcomes.*

I confirm that I will update Kiwi Vet Behaviour if I or anyone in my household are showing any symptoms of COVID19 or return a positive COVID test prior to any appointments.*

I confirm that, to the best of my knowledge, myself or my pet have NOT been exposed to distemper, ringworm, kennel cough, leptospirosis, parvovirus or PBFD within the last 14 days.*

I agree to follow the relevant health and safety protocols when on-site at the Kiwi Vet Behaviour centre.*

I understand that Kiwi Vet Behaviour reserves the right to refuse service to myself and my pet at any time for any reason.*

I understand that to ensure the safety and effectiveness of any training programs, Kiwi Vet Behaviour reserves the right to cancel or reschedule any training sessions if we deem conditions unsuitable. This includes but is not limited to severe weather conditions.*

I agree to allow images and videos of my pet to be taken for educational/promotional purposes for Kiwi Vet Behaviour’s website and social media platforms. I understand that as images can be downloaded from the internet, Kiwi Vet Behaviour is not responsible for any unauthorised use of these images.
